Direct Primary Care is a membership-based model of healthcare. Instead of billing insurance for visits, patients pay a flat monthly fee for access to care. Lumen Integrative Health is a membership which covers visits, messaging access, and ongoing follow-up. Individual memberships are $99/month or a couple membership is $149/month. For non-members, new patient appointment is $250 (required) with follow ups $150 per visit and urgent care is $75 per visit.

Lumen DPC is not an insurance plan. While our practice is designed to offer a full range of primary care services that cut down on referrals and visits, we do recommend all our patients carry medical cost-sharing memberships or insurance to cover unexpected surgery, hospitalization, or ER visits.

Yes. Individual visits are available for patients who prefer not to enroll in a Direct Primary Care membership. Individual visit options include: • New Patient Visit (required): $250 • Follow-Up Visit: $150 • Urgent Care Visit: $75 However, integrative medicine, medical weight loss, and hormone therapy services require an active membership. These services involve ongoing evaluation, monitoring, and continuity of care, which are best supported within the Direct Primary Care model. Membership ensures appropriate follow-up, lab review, medication management, and long-term oversight to provide safe and comprehensive care.

Lumen does not accept or bill insurance for visits. However, you can still use your insurance for labs, imaging, and medications. If you have an HSA or FSA, you may be able to use those funds for your membership fee as well.

Yes. Lumen Integrative Health is a telehealth-only practice serving adults ages 18–64 in Georgia and Florida. Telehealth allows for flexible scheduling, easier follow-up, and more consistent communication without sacrificing quality of care.

Your monthly membership includes: • Telehealth visits • Annual wellness visits • Preventive care planning • Chronic disease management • Acute/urgent visits • Medication management • Lab review & interpretation • Lifestyle counseling • Secure communication access Labs, imaging, and medications are not included in the membership fee.

Lumen serves adults ages 18–64. Due to federal regulations regarding Medicare, we are unable to enroll Medicare patients at this time.
